Application for Degree or Certificate

All candidates for a degree or certificate must submit an Application for Degree/Certificate (DocuSign) during their last semester of coursework. This application is the candidate’s formal request to the Registrar’s Office for an audit of his or her academic record to determine eligibility for a degree and/or certificate from Jefferson. This form must be fully completed, signed, and mailed to the Jefferson Community College Registrar's Office.
